• Children (13 and under): $199/year

• Families: $350/year for first member, $199 for each additional family member

Please note that this membership plan is not an insurance plan and cannot be combined with any dental insurance or discount programs. Coverage is exclusive to services provided at our office and is non-refundable and non-transferable. The plan automatically renews each year unless canceled by the member.
